Application Instructions

Welcome! to the Horizon Transport contractor application website. You will be using this site to submit and track your contractor application with Horizon Transport. Before you begin please be sure to read the following instructions carefully:

It will take you approximately 20 to 30 minutes to complete your contractor application. You may partially complete the application, save it and login back again to finish your application at a later time. You may return to your partially completed application as many times as needed to complete and submit it. Once the application has been submitted, it cannot be modified. All the required information must be provided to submit your application for approval. Following items are required for submitting the application.

Filling out this online application involves navigating between multiple screens with many text inputs. Therefore, we recommend you use at a minimum full size tablet computer but preferably a laptop or desktop computer.


We recommend that you gather all these required information before you start working on the application.

  1. Decide on which divison you would like to apply. We serve Drive Away, Tow Away, Haul N Tow and Truck and Trailer Divisions. If you are not familiar with these divisions, please click here to learn more before you start.
  2. Your personal information such as Current address, Home Phone, Cell Phone, SSN, Date Of Birth
  3. All your previous addresses for the past 3 years
  4. Emergency Contact information. (we need at least two emergency contacts). These contacts will be used in case of an emergency. We need their Name, Contact number and your relationship with them.
  5. Your Driving License information - We need License number, State that it was issued at, License Type and expiration date. We also need information of licenses from other states over the past 3 years. We also require information regarding any suspension, revokation or denial of license at any time.
  6. Your Driving Experience. We need information regarding your past and present driving experience regarding Class of Equipment, Type of Equipment and approximate driving miles on those equipment.
  7. Your Vehicle Information - Year, Make, Model, VIN and Gross Vehicle Weight Rating (GVWR). If the year of the vehicle is older than 2000, we need pictures of all four sides. No Vehicle Information is needed if you are applying only for Drive Away division.
  8. Your Work History - To comply with the DOT requirements, we need your work history for the past 10 years. We need reference contact information for all your work history in the past 3 years. Your work history can not have any gaps. If you have been unemployed, retired, in school, medical or any other reason you need to provide that information in the work history section. Important: The most common cause for delayed application approval is missing work history information. We can not approve your application with work history gaps.
